What to do now

1. Check your email for a message from FollowMyHealth
   (This is our secure patient portal)

2. Open the email and create your account
   When asked for an invite code, use your date of birth (MMDDYYYY)
   Example: 01051990 — no slashes

3. Once logged in, go to the “My Health” tab and complete:

   • Medications

   • Allergies

   • Medical conditions

That’s it!

Once this is complete, you’ll be able to message your clinician securely through the portal.
